HealthCasa brings regulated health professionals to your patients’ homes, or sees them virtually, across Ontario and in Calgary and Edmonton. Download our referral form for your EMR. It fills in your patient’s details, you tick the services they need, and you print it for your patient or fax it to us.
An eForm that auto-fills patient details plus your name, CPSO #, billing # and clinic contact info from the chart.
Unzip the file first (on Windows, right-click it and choose Extract All; on a Mac, double-click it). In OSCAR’s administration menu, open the eForms section, choose Upload an eForm, select the extracted .html file, and name it “HealthCasa Physician Referral”. Then open any patient chart, go to eForms, and choose HealthCasa Physician Referral. Tick the services, add a diagnosis, then Print, or Save to chart.
In Accuro, go to Tools > Form Editor > Create a New Form > From File and choose this PDF. Add Patient and Provider fields over the blank lines so they fill in from the chart, then save.

In PS Suite, go to Settings > Edit Custom Forms, then File > Import Form(s), and choose the extracted .cfm file. If asked, choose to update the existing form. Open it from any patient chart and type X beside each service needed.
