Most insurance plans do provide some coverage for our Services, including the following: Registered Massage Therapy, Physiotherapy, Chiropody, Naturopathy, Dietetics, Custom Made Orthotics and Compression Socks. 

Every insurance plan is different, so we strongly suggest contacting your insurance company before your appointment

to determine exactly what is covered by your plan.

What Do I Submit?

 

Making insurance claims can be a confusing process, that’s why we’ve created this handy guideline for you to make things easier when submitting all your documents:

Assessments and Treatments (Massage, Physiotherapy, Chiropody, Naturopathy and Dietetics

1. HealthCasa Appointment Receipt

This is like your credit card slip, similar to what you’d get at a store and is emailed to you after your appointment


 

Orthotics Appointments

(5 documents)

1. HealthCasa Appointment Receipt

This is like your credit card slip, similar to what you’d get at a store and is emailed to you after your appointment.

 

2. Orthotics Prescription (Rx-OT)

This is your orthotics prescription created for you by your podiatrist/chiropodist. This is emailed to you after your receive your orthotics. 

 

3. Biomechanical Assessment/Gait Analysis Report (BA)

This is emailed to you after your receive your orthotics. 

 

4. HealthCasa Orthotics Receipt

This is emailed to you after your receive your orthotics. 

 

5. Lab Report & Lab Invoice (2 page document)

This is emailed to you after your receive your orthotics. 

 

*****Please Note that, as per insurance requirements, you cannot submit your claim for custom orthotics until they have been dispensed (i.e. when you receive them)
